Hello & welcome to the Beautyholic Terms of service!

This Liability Waiver and Release Form (the "Form") is designed to release The BeautyHolic, its owner Sonia Felix, and employees from liability for any injuries, damages, or claims arising from the aesthetic and wellness treatments provided.

I, the undersigned, acknowledge that I have voluntarily chosen to undergo aesthetic and wellness treatments provided by The BeautyHolic. I understand that these treatments, which may include but are not limited to Skin treatments, body contouring, vaginal rejuvenation, skin bleaching, teeth whitening, fat reduction, plasma fibroblast, skin tag removal, scar removal, microneedling, and other aesthetic and wellness procedures, involve risks such as, but not limited to, local pain, skin redness (erythema), swelling (edema), damage to the natural skin texture (crust, blister, burn), change of skin pigmentation (hyper- or hypo-pigmentation), and scarring.

I fully understand that there are risks associated with the treatments provided and I voluntarily assume full responsibility for any risk of injury, loss, or damage arising out of or related to the treatments provided by The BeautyHolic.

In consideration of being permitted to receive treatments at The Beauty Holic, I hereby release, waive, and discharge The BeautyHolic, its owner Sonia Felix, and employees from any and all liability, claims, demands, actions, or causes of action arising out of or related to any loss, damage, or injury, including death, that may be sustained by me or to any property belonging to me, while participating in the treatments, or while on the premises where the treatments are being conducted.

I agree to indemnify and hold harmless The Beauty Holic, its owner Sonia Felix, and employees from any and all claims, actions, suits, procedures, costs, expenses, damages, and liabilities, including attorney's fees, arising out of or related to my participation in the treatments.

This Form shall be binding upon me, my heirs, executors, administrators, successors, and assigns.

I affirm that I am of legal age in the state of California and am freely signing this Form. I have read this Form and fully understand that by signing this Form, I am giving up legal rights and remedies that may be available to me.

This Form shall be governed by and construed in accordance with the laws of the state of California.

I acknowledge that I have read and understood the above information, and I voluntarily agree to the terms and conditions stated above.
